Job Title:LVN Treatment/Wound Care Nurse Location:Culver West Health CenterJob Type:Full-Time / Part-TimeReports To:Director of Nursing (DON) Job Summary:We are seeking a compassionate, skilled, and detail-oriented Licensed Vocational Nurse (LVN) Treatment/Wound Care Nurse to join our clinical team at our skilled nursing facility in California.The ideal candidate will be responsible for delivering high-quality wound and skin care to our residents in accordance with physician orders, nursing standards, state and federal regulations, and facility policies.This position plays a vital role in promoting resident healing, preventing skin breakdown, and ensuring compliance with treatment protocols.Key Responsibilities:Provide direct wound and skin care treatments as prescribed by the physician and in accordance with nursing standards.Conduct regular and thorough skin assessments and wound evaluations, including measurements, staging, documentation, and photographing (if required).Collaborate with interdisciplinary teams including physicians, RNs, physical therapists, dietitians, and other healthcare professionals to develop and implement individualized treatment plans.Monitor progress of wound healing and notify appropriate personnel of changes in condition.Ensure timely and accurate documentation in the electronic medical record (EMR) system per facility policy and regulatory guidelines.Maintain an updated treatment log and skin report.Assist with pressure injury prevention programs and conduct staff education as needed.Follow infection control protocols and ensure that all treatments are provided in a clean and safe manner.Participate in quality assurance (QA) initiatives related to wound care and skin integrity.Attend care plan meetings and contribute to resident-centered care planning.Ensure compliance with California Department of Public Health (CDPH) regulations, CMS guidelines, and facility policies and procedures.Qualifications:Current and active Licensed Vocational Nurse (LVN) license in the State of California.Minimum of 1 year of experience in wound care/treatment nursing in a skilled nursing or long-term care setting preferred.Certification in wound care (e.g., WCC, WOCN) is a plus but not required.Current CPR/BLS certification.Strong assessment and clinical skills, with attention to detail.Familiarity with electronic medical records (EMR) systems.Ability to work effectively with residents, families, and interdisciplinary teams.Knowledge of state and federal regulations governing skilled nursing facilities.Excellent organizational, communication, and time management skills.Job Type:Full-time Pay:$40.00 - $45.00 per hour Benefits:401(k) Dental insurance Health insurance Vision insurance Experience:Wound care:1 year (Required) Treatment Nurse in a Nursing Home:1 year (Required) License/Certification:LVN (Required) Ability to Commute:Los Angeles, CA 90066 (Required) Work Location:In person.
