Key Responsibilities Collaborate with healthcare professionals to deliver social work case management services Assess and address members' psychosocial needs while coordinating care and resources Document interactions and maintain compliance with accreditation standards and policies Required Qualifications and Education Master's degree in Social Work, Psychology, Education, Public Health, or related field Current state licensure as a social worker or in a related field Minimum of three years of experience with individuals having complex medical, behavioral, and psychosocial needs Experience in patient education and care management Knowledge of chronic health conditions is preferred
