The Spokane VA Healthcare System's Geriatrics & Extended Care Service is seeking an experienced and dynamic Geriatrics Board Certified Attending Physician (Geriatrics) for our Spokane Medical Director, Community Living Center. This position offers a chance to serve our nation's veterans along with excellent work life balance. The CLC provides Nursing Home level of care to Veterans who need short term rehabilitation or skilled nursing care, long term care, dementia care or hospice care. The Community Living Center (CLC) Medical Director is responsible for delivering patient care directly or through the supervision of CLC assigned providers and any assigned medical trainees in both inpatient and outpatient settings. This role entails providing care to adults with acute and chronic medical conditions across a continuum of CLC services, including skilled nursing, long-term care, hospice/palliative care, including pain management, and rehabilitation. The incumbent must evaluate and manage adults with single or multiple disorders across various subspecialties, with a particular focus on geriatric medicine. Moreover, the Medical Director must be proficient in working within interdisciplinary teams to ensure comprehensive patient care. The Physician - Community Living Center (CLC) is responsible for the overall quality of medical care and treatment to the Veteran population as assigned. The physician serves as the primary point of accountability in Geriatrics and Extended Care, Community Living Centers, and maximizes the efficient use of available resources to treat assigned population(s). The Physician (Geriatrics) Community Living Center will have the following direct clinical responsibilities: Reviewing Patients Histories, examinations, assessments and plans for CLC residents coordinating all evaluations, diagnostic testing, clinical follow up, and care coordination. Coordinating of Community Living Center (CLC) Team in the day-to-day management of Veterans. Promoting an atmosphere of efficiency and accuracy in the timeliness of submission of completed documents. Collaborating with Nurse Practitioners/Physician Assistant, ancillary health care providers, and various clinical Specialties. Help facilitate hospital admissions, post discharge care and specialty consultations as needed. Participates in screening of veterans and completion of consults for the CLC. Participates in call schedule for telephonic coverage for urgent needs of veterans in the CLC after administrative hours. Participation in interdisciplinary Team meetings. Participates in other clinical duties as assigned by the ACOS for Geriatrics & Extended Care. VA offers a comprehensive total rewards package. VHA Physician Total Rewards.
