Details Home health setting serving patients in Alameda or Contra Costa County, CA Day shift schedule, 5x8-hour shifts (9:00 AM – 5:00 PM) Weekends required on an as-needed basis No on-call requirement Uses Kinnser/WellSky EMR; training provided Requirements Minimum 1 year of recent home health PT experience required OASIS experience required Active CA PT license required; license must be in hand at time of consideration Current BLS certification required Additional Information ~ Responsibilities include physical therapy evaluation and treatment of patients in a home health setting across Alameda or Contra Costa County ~2 professional references from a supervisor within the past 2 years required for consideration ~ Full 7-year work history with all gaps explained required for consideration ~ All time-off and scheduling requests must be submitted at the time of initial consideration ~ License required if applicable American Traveler Job ID #P-705242.
