Job Description
This position will travel to see patients in the Portland Metro and Clark County (Washington) area. The Hospice Access Field and Triage RN will complete medical requirements while coordinating a whole-person plan of care. Care is according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. In Field role, the RN will visit patients in their homes (private or facility) to present the hospice program and conduct eligibility assessments, coordinating care with Hospice MD, family and facility caregivers, and interdisciplinary team (IDT). In the Triage role, RNs answer calls from patients and families from the Providence Home Services building in NE Portland.
