Mental Health Professional

Supreme Living is looking for a dynamic Mental Health Professional. This candidate must be a licensed therapist. That license could be any of the following: LMHCA,LMHC,LMFT,LASW,LISW. The pay range for this position is $46.00 - $50.00. The therapist will be paid for the entire time that they are working. Job Responsibilities: In coordination with management, the MHP may conduct perspective resident screening interviews and documentation reviews. In coordination with management the MHP conducts an initial Comprehensive Intake Assessment with clients. Develops Service/Treatment Plans for clients outlining specific program goals and objectives. In cooperation with nursing staff, the MHP facilitates weekly Treatment Team meetings, when appropriate. Provides resource information and referrals to assist clients in developing community support systems such as developing supportive relationships, self-advocacy, stapble hoursing, education, and employment. Regularly monitors resident Service/Treatment Plans and updates accordingly. Conducts individual and group therapy sessions as assigned. Conducts educational workshops and groups as indicated. Creates individualized crisis plans for clients as needed during crisis periods. Monitors clients to ensure their safety with the facility, when appropriate. Observes and document behavioral changes in clients. Acts as a liaison with medical professionals and behavioral health professionals providing services for clients. Responsible for accurate and timely charting in the EMR using accepted abbreviations and terminology.
