This vital role offers the opportunity to deliver expert, compassionate care to diverse patient populations, focusing on prevention, diagnosis, and management of infectious diseases. You will play a key role in promoting health education, conducting clinical assessments, and providing tailored treatment plans that improve patient outcomes. Providers are expected to see 25 patients per day and be part of an innovative environment that values proactive healthcare delivery and empowers you to make a meaningful difference in patients’ lives. Responsibilities PROVIDERS are required to love working with patients and foster relationships where patients trust and love coming back to their visits. Provider will meet 25 patient encounters each day through office, telemedicine visits, or outreach events. Provider must love working with the community and support outreach events. Provider must be organized and have great follow up skills with patients and office workflows. Conduct thorough patient assessments, including history-taking, physical examinations, and vital signs monitoring to evaluate health status. Diagnose and treat HIV, sexually transmitted infections (STIs), and other primary care conditions with evidence-based practices. Develop individualized care plans, including medication management, counseling, and health education tailored to each patient’s needs. Administer injections, perform venipuncture, and manage IV infusions when necessary for treatment or diagnostic purposes. Utilize electronic health record (EHR) systems such as eClinicalWorks to document clinical encounters accurately and efficiently. Provide patient education on prevention strategies, medication adherence, and lifestyle modifications to promote overall wellness. Collaborate with multidisciplinary teams for case management, follow-up care, and referral coordination to ensure comprehensive patient support. Requirements Valid Family Nurse Practitioner (FNP) license with certification to practice independently in your state. Proven experience in primary care settings with a focus on infectious disease management or HIV/STD treatments preferred. Familiarity with EMR/EHR systems such as Epic or Athenahealth for documentation and clinical workflows. Strong clinical skills including performing physical exams, suturing, phlebotomy, and sterile technique procedures. Knowledge of CPT coding, ICD-10 coding standards for accurate billing and documentation. Experience working with diverse populations including children, seniors, individuals with disabilities or developmental disabilities is highly desirable. Critical care or hospital experience involving acute pain management, triage, or emergency medicine is a plus.
