You will travel to clients’ homes, perform thorough quality-of-life assessments, guide families through difficult decisions, and deliver peaceful, dignified farewells. Responsibilities include detailed medical documentation, medication management, and maintaining high standards of safety and professionalism. You will collaborate closely with a supportive telehealth and care coordination team, offering both clinical expertise and emotional support.
