These seven words define Rite Med Group.We see you as more than a provider--you're a person with goals, ideas, and purpose.From your first interview to your daily practice, you'll be supported as a valued member of our care team.We advocate for you and help create a work environment that fits your professional and personal needs.Position Overview:As a Neurology Nurse Practitioner (NP), you will provide specialized neurological care to patients across assisted living settings throughout Queens, Long Island, Bronx and Brooklyn.This role focuses on delivering high-quality, patient-centered neurological care while collaborating closely with interdisciplinary teams.The ideal candidate is detail-oriented, proactive, and passionate about improving neurological health and reducing acute neurological events in geriatric and post-acute populations.Duties & Responsibilities:Provide on-site neurological evaluations, diagnosis, and management for patients in assisted living, skilled nursing, and long-term care facilities Assess and manage common neurological conditions such as dementia, Alzheimer's disease, Parkinson's disease, stroke, neuropathy, seizures, and chronic headaches Monitor and manage patients with chronic neurological conditions, including medication adjustments and ongoing symptom management Interpret diagnostic information such as neurological exams, imaging reports (CT/MRI), vital trends, and relevant lab results to guide treatment plans Prescribe and manage neurological medications, ensuring safe and effective treatment while monitoring for side effects and interactions Collaborate with interdisciplinary care teams, including physicians, nursing staff, and facility leadership, to develop and implement individualized care plans Provide early intervention to prevent hospitalizations and reduce neurological complications such as falls, cognitive decline, and stroke recurrence Educate patients, families, and facility staff on neurological conditions, disease progression, prevention strategies, and treatment adherence Maintain accurate, timely, and compliant medical documentation in accordance with organizational and regulatory standards Ensure compliance with all state, federal, and facility-specific healthcare regulations Support continuity of care by coordinating follow-ups, referrals to neurologists when necessary, and ongoing treatment plans Job Types:Full-time, Part-time, Contract Pay:$155,000.00 - $195,000.00 per year Application Question(s):How soon are you able to start a new position?
