Department: Primary Care-Adv Pract Prov FTE: 1.00 Full Time Shift: Days Position Summary Function in a primary care setting in the role of practitioner, mentor, consultant, educator, researcher and manager. Establish goals and priorities in accordance with patient and staff needs and organizational objectives. Ensure adherence to Hospitals and departmental policies and procedures. Patient care - Women's Specialties: Ages of patients served restricted to adolescent, adult and geriatric age groups. Family Practice Clinic: Ages of patients served may include neonate, pediatric, adolescent, adult and geriatric age groups. Senior Health Clinic: Ages of patients served restricted to geriatric age groups. Detailed responsibilities PATIENT CENTERED MED - Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable PATIENT CARE - Assess, diagnose and develop treatment plans; implement treatment, follow-up and evaluate patient within the scope of practice protocols as outlined by state statute PATIENT CARE - Evaluate and revise intervention care/treatment plan with patient and family as necessary to achieve improved health PATIENT CARE - Provide education for patients and/or families about preventative care, medical issues and use of the prescribed medical treatment DOCUMENTATION - Provide accurate documentation of patient care, document orders, directives, p
