Job Description
As a Nurse Case Manager at Incredible Health, you will leverage your clinical expertise and passion for patient advocacy to drive tech-enabled, high-quality care coordination across healthcare systems. Working within the Healthcare & Medical Services department, you will use Incredible Health’s innovative digital platform to streamline communication between patients, providers, and care teams, helping health systems improve outcomes, reduce readmissions, and enhance the overall care experience. In this role, you will conduct comprehensive clinical assessments, review medical records, and identify patient needs across the continuum of care, including acute, post-acute, and community settings. You will develop and maintain individualized care plans, focusing on evidence-based interventions, safe transitions of care, and timely follow-up. Partnering closely with physicians, bedside nurses, social workers, therapists, and payers, you will coordinate services such as home health, rehabilitation, durable medical equipment, and community resources to support optimal recovery and long-term health. You will perform utilization review to help ensure that patients receive medically appropriate and cost-effective care, applying established clinical criteria and payer guidelines. Using your knowledge of insurance processes, you will collaborate with case management teams and payers to support authorization, length-of-stay management, and discharge planning. Your work will emphasize both quality and efficiency, reflecting Incredible Health’s mission to modernize healthcare operations through technology-driven solutions. You will also serve as a clinical resource on case management best practices, supporting both partner facilities and internal teams. This position is ideal for an experienced registered nurse who enjoys balancing direct clinical insight with strategic problem-solving, communication, and technology. You should be comfortable managing a diverse caseload, prioritizing tasks, and working semi-independently while remaining highly collaborative with multi-disciplinary colleagues. Strong communication skills, empathy, cultural sensitivity, and a patient-centered mindset are essential, as you will frequently educate patients and families about diagnoses, treatment plans, medications, and community support options.
