Description The University of Iowa Health Care, Care Coordination Division is seeking a Nurse Clinician to function as a clinical nurse expert within the Medical Surgical Services - Inpatient units and care coordinator for the assigned patient population facilitating seamless transitions across the continuum of care. The nurse functions as an administrative and clinical liaison for individuals to optimize quality health care delivery using clinical assessment/screening tools, education reinforcement, follow-up on the plan of care recommendations and guiding the utilization of cost-effective resources. This position will manage a specified population and work closely with other care coordinators during transitions of care as well as other interdisciplinary team members across the Continuum of Care. Duties to Include: Collaborate with nursing staff and other health care professionals in the development and review of standards and in planning, implementing and evaluating patient care for a specific patient population. Communicate with physicians and members of the health care team directing patient care. Provide direct nursing care, facilitate continuity of care and follow universal precautions. Coordinate and facilitate patient care between multiple disciplines including home health agencies, referring physicians and other community support services. Coordinate patient care to better utilize supportive services through patient visit and possible admission, pre-schedules testing, follows up with test results to both patient and physician, obtains medical history, coordinates follow up visit appointment, provides medication, rehabilitation teaching, etc. Serve as a clinical resource or consultant for specific patient population.
