FTE: 1.0, Shift: Days, Schedule: Mon-Fri Position Summary The Nurse Navigator Acute Care is a multi-faceted position providing comprehensive services to patients throughout the acute care continuum working in the role of registered nurse, case manager, and patient/physician liaison. This position is responsible for the clinical, quality, and cost effective outcomes for a caseload of patients, and for collecting, trending, and analyzing aggregate outcomes data for the patient population to be used in identifying best practices and targeting performance improvement efforts. This position requires the advanced knowledge and expertise to manage cardiovascular hospitalized patients, understand triage principles, and identify and implement improvement processes. Work situations require substantial clinical competency and specialized knowledge, judgment, and skill based upon the physical, developmental, psychological, environmental and spiritual principles. This position requires self-direction, motivation, organization, and interpretation of information relating to policies, procedures, and programs. Internal and external contacts include nursing staff and management, physicians, patients, families, community agencies, and professional peers. Requirements Bachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service may be considered
