Nurse Navigator

Wesley Woodlawn Hospital & ER seeks a Nurse Navigator to guide patients and families through the care continuum in our community-focused, acute care setting. This role coordinates appointments, referrals and transitions across inpatient and outpatient services, ensuring timely, patient-centered care. The Nurse Navigator provides clinical assessment, education and advocacy, collaborating closely with physicians and the interdisciplinary team.
