In addition, the group provides outpatient surgery and endoscopy, imaging, cardiac testing, and infusion services. We are seeking a full-time Nurse Practitioner (40 hours per week) to join our Family Medicine practice at 21 Elm Street in South Deerfield, MA. Our hours of operation are Monday–Friday, 8:00am–5:00pm, with a weekly schedule of 36 hours of direct patient care and 4 hours of administrative time, designated on Fridays. Our South Deerfield office is a close-knit, community-centered practice dedicated to comprehensive primary care for patients of all ages. We provide physical exams, routine screenings, preventive care, and the diagnosis and management of acute and chronic conditions—with a strong focus on prevention and long-term wellness. Primary care is about relationships, and we take pride in truly knowing our patients to deliver exceptional, personalized care. This role will support a patient panel of approximately 1,000 patients. The NP is responsible for assessing and managing various patient populations, including those with medical and/or surgical conditions, as well as providing diagnostic and therapeutic interventions, developing appropriate plans of care, and conducting ongoing evaluations. Does this position require Patient Care? Yes Essential Functions Provides direct care, counseling, and education to a designated patient population in ambulatory, inpatient, operative, and/or procedural settings. Performs complete histories and physical examinations. Orders, interprets, and evaluates appropriate laboratory and diagnostic tests. Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings. Consistently provides high-quality and timely documentation, including admission and progress notes, procedure notes, operative notes, and discharge summaries. Performs bedside procedures as are appropriate to the patient population. Orders medications and writes prescriptions in accordance with organizational and regulatory policies and procedures.
