Job Description
Aachele Home Care and Hospice Cobb County, Georgia Job Summary Aachele Home Care and Hospice is seeking a compassionate and experienced Nurse Practitioner (NP) to provide high-quality palliative care services to patients in the Cobb County area. The Nurse Practitioner will work collaboratively with physicians, nurses, social workers, and interdisciplinary team members to deliver patient-centered care focused on symptom management, quality of life, and support for patients with serious or chronic illnesses. Responsibilities Conduct comprehensive patient assessments in homes, facilities, or designated care settings Develop and implement individualized palliative care treatment plans Manage pain and symptoms related to chronic and serious illnesses Order, perform, and interpret diagnostic tests as appropriate Prescribe medications and treatments according to state regulations and clinical protocols Educate patients and families regarding diagnoses, treatment options, and disease progression Collaborate with physicians, hospice teams, case managers, and community providers Maintain accurate and timely documentation in electronic medical records Participate in interdisciplinary team meetings and care coordination Provide emotional support and advocacy for patients and caregivers Ensure compliance with state, federal, and company regulations and standards Qualifications Active Georgia Nurse Practitioner license Master’s or Doctorate degree in Nursing from an accredited program Board Certification as a Nurse Practitioner (FNP, AGNP, or related specialty) DEA license and prescriptive authority in Georgia Minimum of 1–2 years of experience in palliative care, hospice, geriatrics, oncology, or home health preferred Strong clinical assessment and communication skills Ability to work independently and manage a flexible schedule Current CPR certification Valid driver’s license and reliable transportation Preferred Skills Experience with home-based care Knowledge of Medicare and Medicaid regulations EMR documentation experience Strong interpersonal and family communication skills Compassionate bedside manner and patient advocacy mindset Work Schedule Full-Time or Part-Time opportunities available Flexible scheduling based on patient needs Benefits Competitive salary Flexible work schedule Mileage reimbursement Continuing education opportunities Supportive interdisciplinary team environment Service Area Primary coverage area includes Cobb County and surrounding Metro Atlanta communities.
