Duties Conduct comprehensive psychosocial assessments to identify residents' needs and develop personalized care plans.Provide crisis intervention and crisis management support to residents experiencing emotional or behavioral distress.Assist residents with navigating healthcare options such as Medicare, Medicaid (Section 8), and other community resources.Collaborate with interdisciplinary teams to coordinate services related to behavioral health, addiction counseling, and child welfare when applicable.Offer counseling and motivational interviewing to support residents' mental health, behavioral changes, or transitions.Supervise and mentor junior staff or interns, fostering professional growth within the social work team.Advocate for residents' rights and ensure compliance with healthcare regulations while promoting resident-centered care.Qualifications Bachelor's or Master's degree in Social Work (BSW or MSW) from an accredited program.Valid licensure or certification as a Licensed Clinical Social Worker (LCSW) or equivalent preferred.Proven experience in social work within nursing homes, behavioral health settings, or related environments.Strong skills in crisis intervention, crisis management, and behavioral health support.Familiarity with addiction counseling techniques and motivational interviewing methods.Experience supervising staff or mentoring students/interns is highly desirable.Knowledge of healthcare programs including Medicare, Medicaid, Child Welfare, and Workforce Development initiatives.Join us in creating a caring community where every resident receives the support they deserve!
