GEM Transition Care Center is a 75 bed skilled nursing facility specializing in short-term therapy and dedicated to providing all of our residents with the type of care they deserve. We are looking for a full-time Occupational Therapist to join our growing in-house team. We offer a variety of therapies and care services to assist patients in regaining their independence, whether they are recovering from a hospital stay or require ongoing medical support. As the Occupational Therapist and a valued team member, you will collaborate with therapists, patients, their families and the IDT to guide each patient through a personalized therapy program, helping them reach their recovery goals. Good physical and mental health. Capability to perform all physical duties of the position, including lifting up to 40 pounds unassisted. Ability to follow oral and written instructions. Possess training, knowledge, and experience for general supervision responsibilities. REPORTING The Occupational Therapist reports to the Rehabilitation Director. JOB DUTIES Understanding of physical medicine, occupational therapy modalities, anatomy, and physiology. Basic knowledge of operating occupational therapy machines, as well as their maintenance and repair. Administering treatment and rehabilitation services to patients with injuries or disabilities. Conducting exercises for residents. Adherence to physical medicine practice standards. Observing and assessing treatment effects, discussing observations with staff and Director of Rehab Monitoring and documenting resident treatment and responses. Assist in developing a teaching plan based on learning needs and evaluating its effectiveness. Involve the family in teaching when appropriate. Inform staff and the Director of Rehab of outcomes.
