MEDSTAR HEALTH is seeking an Occupational Therapist for Home Care services in Silver Spring to join our Healthcare & Medical Services team. As a key member of a patient-first, not-for-profit healthcare organization serving the D.C.-Maryland-Virginia region, you will provide skilled, evidence-based occupational therapy in patients’ homes, helping them regain or maintain independence, safety, and quality of life where they live. In this field-based role, you will conduct comprehensive evaluations that address functional status, cognition, psychosocial factors, and the home environment. You will develop individualized plans of care that focus on activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, and safe mobility within the home. Interventions may include ADL retraining, therapeutic exercise, energy conservation strategies, joint protection techniques, cognitive and perceptual rehabilitation, and recommendations for adaptive equipment and durable medical equipment. You will perform detailed home safety and fall risk assessments, identifying environmental barriers and recommending modifications or assistive devices that support safe, independent living. Working within MedStar Health’s collaborative, ethically driven, and innovation-focused culture, you will coordinate closely with physicians, registered nurses, physical and speech therapists, social workers, and case managers to ensure integrated, patient-centered care. You will educate patients and caregivers on safe transfer techniques, use of adaptive equipment, home exercise programs, and strategies to manage chronic conditions and prevent hospital readmissions. Accurate and timely electronic documentation is essential, including evaluations, progress notes, care plans, and discharge summaries that meet home health, Medicare, and payer-specific regulatory standards. You will be expected to manage your own schedule efficiently, traveling throughout the Silver Spring area to provide care in diverse home environments while maintaining high professional and ethical standards.
