Occupational Therapist – Post Acute Care Location: Grand Junction, CO Duration: 16 Weeks Contract Shift: Day Shift – 8 Hour Shifts Hours: 30 Hours/Week Guaranteed We are seeking a compassionate Occupational Therapist to provide rehabilitation services in a post-acute care environment. This role focuses on helping patients regain independence and improve daily living skills. Responsibilities: Evaluate patients and develop occupational therapy treatment plans Assist patients with improving daily living and functional skills Implement therapeutic interventions and adaptive techniques Document patient progress and treatment outcomes Collaborate with rehabilitation and nursing teams Educate patients and caregivers on therapy goals and exercises Requirements: Active Occupational Therapist license in Colorado or compact eligibility required Graduate of an accredited Occupational Therapy program required Strong interpersonal and patient care skills required Preferred: Previous LTC or rehabilitation experience preferred Current BLS certification preferred For more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
