Position General Summary: Occupational therapists develop individualized treatment plans based on the evaluation of patients. Occupational therapists provide direct patient care and delegate appropriately to support staff. Occupational therapists provide ongoing direction to patient care and evaluate outcomes. This position is an Essential position at the facility to ensure efficient operations and patient care. Employees hired into an Essential position are expected to report to work for their scheduled shifts regardless of the environmental weather and road conditions that may exist. Position Duties and Responsibilities: Initiates Occupational therapy services upon referral with appropriate diagnosis. Develops treatment plans including short- and long-term goals. Regularly assesses patient progress status through short-term goal appraisal. Demonstrates knowledge of specific conditions, diseases, and treatment therapies for specific populations. Advises on adaptive equipment to help patients with daily activities. Develops physical rehabilitation programs to help clients regain lost skills. Documents timely and accurately (to include initial evaluations, progress notes, and discharge summaries), and meets requirements of chart audit or third-party review. Provides and monitors care to determine the appropriate outcomes. Utilizes a collaborative interdisciplinary process to provide appropriate patient/family education. Assesses patient/family learning needs, abilities, and readiness to learn, and integrates needs into plan of care. Provides appropriate patient care and clinical information for continuing care based on the patients' needs as needed from time of admission to time of discharge.
