Job Title: Occupational Therapist – Home Health Location: Wallingford CT, 06492 Duration: 3 months with contract extensions Shift: Days, Mon-Fri Description: Weekly Hour Guarantee: 32 On Call/Weekend Requirement: No Coverage Areas: I need 2 positions; 1 for Milford CT and surrounding towns and 1 for Waterbury CT and surrounding towns Will Milage be reimbursed between visits, from first patient to last patient (Y/N)? Yes What are the top responsibilities for this position? (Please be as detailed as possible): Roles & Responsibilities Provide skilled home health services to patients in their homes per physician orders and plan of care Perform patient assessments and deliver safe, effective, compliant care Monitor patient progress and report changes to the care team Educate patients and caregivers on treatment plans and disease management Coordinate care with physicians and interdisciplinary team members Maintain accurate, timely documentation per LHC Group and regulatory standards Travel within assigned coverage area; mileage reimbursed between visits Requirements Home Care/Home Health experience (required) Active Connecticut license (role-dependent) CPR/BLS certification Valid driver’s license and reliable transportation Must have qualifications/experience : Home Care experience Dress code/scrub color : Business casual or scrubs
