We are seeking a dedicated and compassionate Occupational Therapist to help patients with injuries, illnesses, or disabilities regain or improve daily living skills while working in a home health setting in Centralia, WA. Minimum Requirements Doctoral, Master’s, or Bachelor’s degree in Occupational Therapy from an accredited educational program. Active Washington State Occupational Therapy license required. Home Health experience required. Client requires experience with HCHB Home Health documentation. Proof of professional certification as required by the role. BLS certification may be required from AHA or ARC. Experience with patient care assessments, recertifications, and discharges preferred. Business casual dress code. Candidate may be expected to follow client routing and coverage within the service area. Floating to other facilities is not required for this home health assignment.
