Occupational Therapist (Home Health) - Full-Time Job location: Jackson, CA Work Setting: Field-based (patient homes), travel required within assigned service area Key Responsibilities Home Health Patient Care Provide occupational therapy services in patients' homes Evaluate patient needs and develop individualized treatment plans Implement and adjust care plans to support functional independence Educate patients and caregivers on therapy techniques and daily living skills Care Coordination Collaborate with interdisciplinary care teams (PTs, nurses, etc.) Communicate patient progress and updates effectively Participate in case management and care planning Requirements Active Occupational Therapist License (State-specific) OTR certification required Valid driver's license, reliable transportation, and auto insurance Strong clinical, organizational, and communication skills Preferred Qualifications Home Health experience Minimum 1 year of Occupational Therapy experience Compensation & Benefits Competitive pay Health Insurance Dental Insurance Vision Insurance Ongoing training and orientation Continuing education support Why Join?
