The OSO Clinical Intake Specialist is a licensed clinical professional responsible for reviewing and processing referrals received through Internal Referral Portals (IRP) and External Referral Portals (ERP) for Home Health and Hospice services. This role works closely with the OSO Intake Specialist to ensure clinical appropriateness, timely documentation, and accurate entry into the Homecare Homebase (HCHB) EMR system. The Clinical Intake Specialist plays a key role in coordinating care transitions and ensuring regulatory compliance. Review and validate referrals and clinical documentation for appropriateness, eligibility, and regulatory compliance, collaborating with Intake Specialists to ensure accurate and complete intake and timely, accurate data entry across EMR and referral systems. Communicate and collaborate with referral sources, physicians, business development, care coordination, and internal teams to clarify clinical information and coordinate timely patient visits. Required: Education: Associate's Degree in Nursing or equivalent Experience: 1 Year of Clinical Administrative Healthcare Licensure: LPN or equi
