About Us Spark Rehab + Wellness is a locally owned outpatient rehabilitation clinic in Bend, Oregon, serving adults with complex mobility, neurologic, geriatric, and chronic medical conditions. Our team provides individualized rehabilitation for patients who may not fit neatly into a traditional outpatient orthopedic or neurologic model, including individuals with dysautonomia, Postural Orthostatic Tachycardia Syndrome, Ehlers-Danlos syndrome, connective tissue disorders, long COVID, post-viral conditions, chronic illness, balance and mobility limitations, and age-related functional decline. We are looking for a thoughtful, collaborative Physical Therapist who is interested in working with patients who often require individualized pacing, symptom-guided exercise prescription, careful vitals monitoring, joint protection strategies, and a nuanced understanding of dynamic disability and symptom variability. Position Summary Spark Rehab + Wellness is seeking a Physical Therapist to join our team in a role focused on adults with complex chronic conditions, autonomic dysfunction, connective tissue disorders, post-viral syndromes, and related mobility limitations. This position may also include care for individuals with neurologic diagnoses and geriatric rehabilitation needs. This role is well suited for a PT who enjoys clinical reasoning, patient education, interdisciplinary collaboration, and individualized treatment planning. Many patients in this population require a careful balance between therapeutic activity, physiologic monitoring, pacing, energy conservation, and recognition that traditional exercise progression may not be appropriate for all individuals. Treatment is primarily one-on-one and emphasizes high-quality, individualized care rather than high-volume scheduling. Typical visits are 45–60 minutes, with a caseload designed to support thoughtful treatment, patient education, and meaningful functional outcomes. This is a full-time, in-person position, with most therapists working four-9-or-10-hour shifts depending on clinic needs and candidate availability. Our clinic hours are 8:00am–6:00pm Monday-Friday. Our clinic offers resources that support individualized care, including specialized vitals monitoring, a variety of cardiovascular equipment, an underwater treadmill, overhead harness systems, adaptive exercise equipment, and specialized rehab technology. Mentorship is available, and clinicians are encouraged to grow professionally, collaborate with other providers, and contribute to program development. There may also be opportunities to serve as a clinical instructor for physical therapy students for candidates who are interested in teaching and mentorship. Key Responsibilities Evaluate and treat adults with complex chronic conditions, autonomic dysfunction, connective tissue disorders, post-viral conditions, mobility limitations, and related functional impairments. Develop individualized plans of care that consider symptom variability, orthostatic intolerance, fatigue, post-exertional malaise, pain, joint instability, deconditioning, and functional goals. Prescribe activity and exercise using patient-centered, physiologically guided, and symptom-informed approaches. Monitor patient response to activity, including appropriate use of vitals monitoring and clinical observation. Incorporate pacing strategies, energy conservation, joint protection, functional strengthening, cardiovascular conditioning, balance training, gait training, and mobility interventions as clinically appropriate. Recognize when rigorous or traditional exercise progression may not be appropriate and adapt treatment accordingly. Educate patients and families on condition management, activity modification, home programs, symptom response, and strategies to improve daily function. Collaborate with referring providers and other healthcare professionals to support coordinated care. Document patient progress clearly and adjust treatment plans based on patient response, goals, and clinical presentation. Participate in a collaborative team environment focused on high-quality care, clinical reasoning, and continuous learning.
