Description Clarvida Job Description Job Title: Outpatient Therapist (Mental Health Professional) Program(s): Outpatient Clinic /School Based Mental Health State: Pennsylvania Population Served: All populations Reports To: Clinical Supervisor FLSA Status: Non- Exempt SUMMARY This position is responsible for the provision of individual and family therapy sessions for consumers assigned to each caseload. This position also ensures completion of required clinical documentation related to individual caseloads in a timely manner. ESSENTIAL DUTIES AND RESPONSIBILITIES Demonstrates proficiency with assessment skills by completing psychosocial assessments at in-take and in on-going intervals as outlined by program requirements; completing standardized assessments and/or outcome based measures. Developments individualized treatment plans by adhering to applicable county, state and federal standard regulations when developing individualized treatment plans for consumers Provision of therapy to consumers in individually recommended setting by providing therapy in recommended setting by adhering to recommendations outlined in individual consumer treatment plans Provision of treatment of the whole person by providing service in collaboration with all team members involved with the consumer, and with the understanding that treatment of the whole person is crucial Evaluates individualized consumer-drive treatment plan reviews by completing on-going evaluation of efficacy of treatment evaluations, via development of treatment plan reviews Develops transition and discharge planning by evaluating recovery and progress through treatment goals, symptomology Completes all required clinical documentation by completing progress notes per service, including necessary signatures, discharge summaries, incident reports, treatment reviews via el
