Illumia is seeking a compassionate and clinically skilled Palliative Care RN to join our Healthcare & Medical Services team, supporting patients with serious and life-limiting illnesses. In this role, you will provide holistic, patient- and family-centered care focused on comfort, symptom management, and quality of life. Working closely with an interdisciplinary team of physicians, social workers, spiritual care providers, and therapists, you will assess complex needs, develop and implement care plans, and offer education and emotional support to patients and their families across the continuum of care. As a Palliative Care RN at Illumia, you will conduct comprehensive physical and psychosocial assessments, manage pain and other distressing symptoms, administer medications and treatments in accordance with evidence-based protocols, and coordinate care transitions between hospital, home, hospice, and other care settings. You will advocate for patients’ goals of care, facilitate family meetings, support advance care planning, and ensure that care aligns with patients’ values and preferences. Documentation in electronic health records will be timely, accurate, and compliant with regulatory and organizational standards.
