Job Description
We are seeking a Part-Time Inpatient Unit Registered Nurse to join our hospice team.This position consists of two (2) 12-hour shifts per week plus two (2) weekend shifts per month.Shift hours:7:00 AM - 7:30 PM or 7:00 PM - 7:30 AM.The Registered Nurse plans, organizes, and directs hospice care within our inpatient facility.This role requires strong clinical experience and close collaboration with patients, families, and the interdisciplinary team to coordinate compassionate, high-quality end-of-life care.Patient Care Completes comprehensive initial and ongoing assessments of patients and families, including full physical assessment and medical history.Utilizes the nursing process to deliver professional hospice care.Develops, implements, and revises individualized plans of care; regularly re-evaluates patient and caregiver needs and participates in IDT meetings.Establishes nursing diagnoses and creates goal-oriented care plans incorporating palliative interventions, involving patients and families in care planning.Initiates preventive and rehabilitative nursing procedures.Administers medications and treatments as prescribed in the physician's plan of care.Provides patient and family counseling, education, and health instructions.Acts as Case Manager when assigned, coordinating care for designated caseload.Communication & Care Coordination Completes accurate, timely clinical documentation, including pain and symptom management updates.Communicates patient status changes to physicians and obtains orders as needed.Maintains a safe environment for patients.Collaborates with patients, families, caregivers, and the Interdisciplinary Team to ensure continuity of care and emotional support.Participates in interdisciplinary team meetings.Additional Responsibilities Participates in Quality Assurance and Performance Improvement (QAPI) initiatives.Coordinates equipment, supplies, and services needed for patient care.Supervises hospice aides and delegates tasks appropriately.Maintains professional growth through continuing education and in-services.Accepts and manages assigned caseload responsibly.Qualifications Current Illinois Registered Nurse (RN) license.Graduate of an accredited school of nursing.Current CPR certification.Minimum 2 years nursing experience preferred (hospice, home health, or public health strongly preferred).Self-directed with ability to work independently.Strong clinical judgment, communication, and organizational skills.Ability to meet physical demands of the role, including standing, walking, lifting, and patient transfers.Job Types:Part-time, Contract Pay:$45.00 per hour Expected hours:No less than 24 per week Work Location:In person.
