The listed pay range or pay rate reflects compensation for a full-time equivalent (1.0 FTE) position. Actual compensation may differ depending on assigned hours and position status (e.g., part-time). Pay Range $86,528.00 - $113,630.40 Scheduled Weekly Hours 36 Position Overview Shift: 8:00am - 8:30pm (rotating weekends and holidays) Essential Functions Follows up with and intervenes regarding delays in discharge, attends IDR’s on their dedicated campus, and is the overall collector of patient information regarding throughput. Works closely with interdisciplinary care team members including but not limited to logistics, floor staff, LIP’s, Care Management, Therapy Departments, Administration, Nursing Unit Leadership, and others. Oversees the patient flow process in its entirety and ensures completion of tasks resulting in a shorter length of stay and earlier discharges. Provides daily operational oversight and decision making of patient flow throughout Capital Health by collaborating with Patient Access, LIPs, Unit Management and Staff, Ancillary Department Staff, Care Management, Logistic Department staff and other areas as needed to assess, plan, and implements patient flow activities to promote patient and customer satisfaction. Collaborates with physician, case manager, and bed-tracking to ensure timely coordination of discharge and transfer. Communicates and collaborates with physicians regarding transfer and discharge orders. Assists primary Staff RN, as needed, regarding outstanding issues related to patient transfer and discharge. Assesses patient status and acuity to facilitate the efficient movement of patients through the hospital system by prioritizing inpatient admissions, critical care admissions, and internal or external transfers. Serves as an expert clinical resource for patient placement by ensuring smooth functioning of the Logistics Center process to reduce wait times and avoid unnecessary movements of patients. Serves as contact for bed management as it relates to admissions. Ensures throughput on units and compliance with transfer process through management of people and process. Delegates tasks appropriately and employs resources as needed to ensure high quality and efficient care related to patient flow. Identifies and anticipates barriers to discharge and patient flow activities and works to ensure throughput in ED, inpatient units and compliance with transfer process through management of people, metrics and process improvement. Manages patient flow using Logistic Dashboard data, rounding, and daily metrics. Communicates openly and effectively by providing ongoing education to the health care team on the overall patient flow strategy on rationale for patient placement. Uses effective and appropriate communication styles with patients, significant others, visitors, staff and professional colleagues. Promotes team momentum, enthusiasm, and pride. Performs other duties as assigned. Minimum Requirements Education: Graduate of accredited school of nursing. BSN preferred. Experience: Four years of experience in an acute care setting. Previous Telemetry experience required. Critical Care experience preferred. Other Credentials: AHA BLS - Healthcare Provider, Registered Nurse - NJ Knowledge and Skills: Strong verbal and written communication skills. Excellent customer service orientation skills necessary to communicate effectively and utilize effective conflict resolution and negotiation skills to support a cohesive multidisciplinary team and ensure optimal patient outcomes.
