Job Summary Welcome Home Ministries is seeking a compassionate, organized, and mission-driven Peer Recovery Case Manager to provide direct peer support, case management, and recovery advocacy services for men participating in our recovery housing and ministry programs. The Case Manager plays a crucial role in providing support and guidance to individuals facing various challenges, including mental health issues, addiction, and social difficulties. This position involves assessing client needs, developing personalized care plans, and coordinating services to promote overall well-being. The ideal candidate will possess strong interpersonal skills and a commitment to empowering clients through effective case management strategies. This position requires an individual dedicated to making a positive impact on the lives of others through effective case management, strong Christian faith and example. Additionally, a compelling desire, calling and purpose to serve people in recovery. Duties Conduct comprehensive assessments to determine client needs and strengths. Develop and implement individualized care plans in collaboration with clients and other professionals. Provide crisis management and intervention services to clients in urgent situations. Facilitate access to community resources, including mental health counseling, addiction counseling, food assistance, medical care, legal support services and behavioral health services. Utilize motivational interviewing techniques to encourage client engagement and progress. Monitor client progress regularly and adjust care plans as necessary. Maintain accurate documentation of client interactions, assessments, and outcomes. Ability to work some evenings and rotating on-call weekends as needed. Administer alcohol and drug screening as needed. Assist participants in developing individualized Wellness Recovery Action Plans (WRAP). Collaborate with multidisciplinary teams to ensure holistic support for clients. General Responsibilities Support house managers in addressing resident concerns related to safety, wellbeing, and morale. Provide encouragement and guidance to house managers. Attend monthly house manager meetings and senior staff meetings. Participate in staff development meetings and workflow coordination sessions. Offer mentoring support to clients as they navigate their recovery journeys. Help cultivate sponsors and participants for WHM’s Annual Benefit Golf Tournament. Assist with sponsorship and attendee outreach for WHM’s Annual ThanksGIVING Breakfast. Qualifications Strong crisis management abilities to effectively handle urgent situations. Proficient in mental health counseling techniques tailored to individual client needs. Experience with motivational interviewing to foster client motivation and commitment. Background in recovery support services or related fields is highly desirable. Knowledge of behavioral health systems and community resources available for clients. Excellent mentoring skills to guide clients through challenges and promote personal growth. Ability to maintain confidentiality and professional boundaries. Strong organizational and documentation skills. Expertise in crisis intervention strategies for immediate support. Familiarity with addiction counseling practices is a plus. A person in long-term recovery. Knowledgeable in technology is highly desirable. In good health. An effective communicator and team player. Certification Requirements This position requires the employee to pursue and obtain: Certified Peer Recovery Specialist (CPRS) certification Wellness Recovery Action Plan (WRAP) Facilitator certification Supervision This position is jointly supervised by the Founder/Executive Director and Deputy Executive Director. Final authority and direction reside with the Founder/Executive Director.
