Company Overview Communities For People, Inc is dedicated to providing a continuum of care for youth and families involved in foster care and child welfare systems.Our mission is to ensure that every child has the opportunity to live with a permanent, loving family while continuously improving our programs to meet their evolving needs.EOE.Job Summary Conduct an initial diagnostic assessment and provide ongoing individual, family, and/or group therapy to identified clients.Check out our website for more details!www.communities-for-people.org Essential Duties & Responsibilities Conduct an initial diagnostic assessment of each client including a DSM 5 diagnosis Provide ongoing individual, family, and/or group therapy for identified clients Develop and update treatment plans quarterly, identifying specific treatment goals and target dates of attainment Make appropriate referrals for more intensive services as needed Seek consultation and coordination from team members and supervisor on difficult to treat issues Obtain initial and ongoing authorization from insurance carrier(s) Obtain authorization or refusal of authorization to communicate and coordinate with all service providers involved with client, including Primary Care Physicians.Maintain case records to highest standards, including all Intake documentation, Releases of Information, Initial and Ongoing Treatment Plans, and Progress Notes.Individual/group supervision offered weekly, with the expectation of attendance once a month Participate in on-call rotation and provide crisis intervention as needed Provide billing information to Office Manager Experience:At least 2 years of supervised practice Qualifications:Licensure in Social Work or Mental Health Related Field Approved credentialing with insurance carrier Pay:$45.00/hour for pre-licensure $55.00/hour for Independent License Pay:$45.00 - $55.00 per hour Benefits:Employee assistance program Flexible schedule Work Location:In person.
