The Position: The ElderCare Registered Nurse (RN) provides person-centered chronic care management services to seniors through telephonic and virtual care models. This role focuses on chronic disease management, care coordination, patient education, and health promotion to support quality of life and reduce avoidable hospitalizations. Working as part of a multidisciplinary care team, the RN collaborates with providers, caregivers, and community resources to develop and manage individualized care plans, support care transitions, and ensure continuity of care for program participants and their families. Why Choose Us: Wellness Benefits: Medical, dental, vision, life insurance, and 401k with generous employer match Paid Time Off: Extensive PTO including vacation, sick leave, and holidays Professional Growth: Tuition reimbursement through Snowline Scholars Program for full-time RNs, Flexible Scheduling Options (including remote work) Positive Team Culture: Collaborative environment that celebrates team joy, connection, and achievement Employee Assistance Program: Promoting trauma-informed care and workplace wellness Market-Competitive Pay: $48.00-$61.00 per hour Schedule: Per diem, remote telehealth-based role, up to approximately 16 hours per week Responsibilities: Clinical Care Conduct comprehensive patient assessments via telephone and virtual visits Develop, implement, and monitor individualized care plans for patients with multiple chronic conditions Provide disease-specific education and self-management support Support medication adherence and health coaching initiatives Monitor patient status and identify changes requiring provider intervention Document accurately, timely, and thoroughly in the electronic medical record Participate in care planning discussions and interdisciplinary team meetings Care Coordination Coordinate care across providers, caregivers, and community resources Support transitions of care following hospitalizations or emergency visits Serve as a consistent point of contact for patients and families Assist with advance care planning conversations as appropriate Program Support and Quality Ensure compliance with CMS requirements for CCM and related programs Support quality improvement initiatives and workflow refinement Maintain working knowledge of chronic care management best practices Regulatory Compliance and Quality Assurance Ensure compliance with organizational policies, HIPAA, and applicable regulations Maintain current knowledge of chronic disease management and geriatric care trends Support accurate documentation for billing and quality reporting Requirements Graduate of an accredited School of Nursing Current and active California RN license in good standing Minimum 3 to 5 years of nursing experience in chronic disease management, geriatrics, home health, or primary care Strong clinical assessment and care coordination skills. Proficiency with EMRs, telehealth platforms, and digital documentation. Excellent communication, organization, and time management skills.
