A PRN nurse is a licensed Registered Nurse who works on an as-needed, per-diem basis rather than a full-time or part-time schedule. This role is ideal for nurses seeking flexibility while making a meaningful impact in the community. The PRN Nurse will serve as a key clinical resource by completing client assessments, reassessments, supervisory visits, and caregiver oversight to help ensure quality, compliance, and continuity of care. Position Summary The PRN Nurse provides intermittent, on-call clinical support including client assessments, care plan development, supervisory visits, and caregiver oversight. This role ensures that all services are delivered in accordance with New Jersey home care regulations and professional nursing standards. This is a non-guaranteed hours position, with scheduling based on agency needs, client census, and nurse availability. 1. PRN RN – Assessment & Supervision Only Focus on client assessments, reassessments, and supervisory visits Ideal for nurses seeking flexible, field-based clinical work only 2. PRN RN – Hybrid Combines field work with clinical support tasks Provides more consistent hours and involvement with the agency Key Responsibilities Conduct initial and ongoing client assessments Develop, review, and update individualized care plans Perform supervisory visits to ensure CHHA compliance with care plans Provide clinical guidance and support to caregivers in the field Monitor and document changes in client condition Ensure compliance with NJ regulations and agency policies Communicate effectively with the care team, including coordinators and office staff Participate in case conferences and care coordination as needed Contribute to training, educational support, or staff development initiatives when applicable Support organizational goals through collaboration, resource management, and ongoing professional development Work Expectations Accept shifts on an as-needed (PRN) basis Ability to cover supervisory visits, reassessments, and urgent clinical needs Flexibility to travel within assigned service areas Maintain professionalism, timeliness, and accurate documentation Ability to work independently with minimal supervision Communicate promptly with the office and clinical leadership team Qualifications Active license in the State of New Jersey Has at least 1 year of nursing experience (home care preferred) Strong assessment and clinical judgment skills Experience supervising caregivers (required) Reliable transportation and valid driver’s license Knowledge of home care regulations and documentation standards Professional, dependable, and comfortable working independently in the field Apply Today Join a team that values your expertise, respects your time, and supports your professional growth.
