TWO Direct Hire Opportunities for Inpatient Oncology/Hematology Pharmacists An established healthcare facility is seeking two Oncology/Hematology inpatient pharmacists to join their team in Naperville, IL. These roles focus on delivering expert medication management within acute care inpatient settings, specifically for patients in malignant hematology, medical oncology, gynecologic oncology, and stem cell transplant (SCT) areas. Location Naperville, Illinois Schedule: Typical inpatient hours with day/evening shifts Rotating weekends Responsibilities: Participate in interdisciplinary rounds to support the design, implementation, and monitoring of oncology medication regimens Review drug therapies for appropriateness concerning dose, regimen, interactions, and cost-effectiveness Reconcile home medications for admitted patients and provide counseling on medication adherence and side effects Verify chemotherapy orders and assist nursing staff with medication order inquiries Provide pharmacokinetic consultations and documentation, particularly around vancomycin and aminoglycosides Monitor, document, and report adverse drug reactions and medication errors Participate in chemotherapy desensitization facilitation and convert medications from IV to oral when appropriate Provide evening shift coverage as needed based on departmental requirements Qualifications: Bachelor of Science in Pharmacy required Illinois pharmacist license or eligibility to obtain Illinois licensure Post Graduate ASHP PGY-2 residency in Hematology/Oncology highly preferred Board Certification in Oncology Pharmacy (BCOP) prior to or within 18 months of hire preferred BLS certification required Strong knowledge of intravenous admixture, drug therapy, chemotherapy, and cellular therapies Abil
