Job Description: This residency program opportunity allows for pharmacists to enhance their skill set in the community and specialty pharmacy setting. The primary residency practice site is a community-based specialty pharmacy in Louisville, Kentucky. The pharmacy team services patients with many disease states including oncology, transplant, chronic inflammatory diseases, HIV and HCV. The community pharmacists at this site work closely with providers, patients, and the community to provide optimal drug therapy for patients. Residents gain insights on management issues that can affect the implementation of patient care services in the community and specialty pharmacy settings. The program is academically affiliated with Sullivan University College of Pharmacy and Health Sciences. The resident will complete a teaching certificate program and will assist in teaching opportunities and precepting experiences for the Sullivan University College of Pharmacy and Health Sciences. The residency program graduates will be leaders in the pharmacy community and will have an active role in advancing community pharmacy practice. A graduate from this residency program would be viewed to possess equivalent experience and skills to seek a pharmacy manager position within a specialty pharmacy location or a traditional community pharmacy practice site. Resident Activities -Provide personalized, comprehensive care to patients with complex conditions such as oncology, transplant, chronic inflammatory disease, hepatitis C, and HIV/AIDS -Assist in delivery of medication education to patients -Perform medication therapy management -Recommend and adminster vaccines -Perform patient centered dispensing -Participate in community service events -Design, conduct, and present a practice based residency project at a professional conference -Develop relationships skills needed in health system pharmacy
