In this full-time role, you'll work closely with primary care providers to ensure improved health outcomes through integrated, patient-centered care working with patients of diverse backgrounds, approximately 30% of which are Spanish-speaking. Key Responsibilities: Collaborate with primary care teams to develop and implement care plans. Perform physical therapy consultations in a primary care setting. Provide compassionate care to patients from diverse backgrounds. Qualifications: Autonomous and adaptable with strong communication skills. Experience working with diverse populations; Spanish proficiency or familiarity with interpreter services is a strong plus.
