Job Overview We are seeking a compassionate and reliable Physical Therapist (PT) to join our growing home health team. This position is flexible and based on patient referrals and client assignments. The Physical Therapist will provide in-home therapy services to patients in their residences to help improve mobility, strength, balance, safety, and overall independence. This is an excellent opportunity for therapists looking for flexible scheduling and supplemental income. Responsibilities Evaluate and assess patients’ physical therapy needs in the home setting Develop individualized treatment plans Provide therapeutic exercises, gait training, balance training, and mobility assistance Educate patients and caregivers on exercises and safety techniques Recommend adaptive equipment and home safety modifications Document patient progress and treatment notes accurately and timely Coordinate care with physicians, nurses, and other healthcare professionals Follow all home health regulations and agency policies Qualifications Current California Physical Therapist (PT) license Minimum 1 year of experience preferred (home health experience is a plus) Current CPR certification Valid driver’s license and reliable transportation Strong communication and documentation skills Ability to work independently and manage schedule efficiently Compensation Paid per visit / per client Flexible schedule based on patient assignments Competitive per-visit rates Schedule Choose your own availability Monday–Saturday Sunday availability optional Work Location Patients are located throughout Los Angeles County and Orange County.
