Overview We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will be responsible for evaluating and treating patients with various physical conditions, developing individualized care plans, and providing high-quality patient care across different settings including inpatient, outpatient, and home health environments. This role requires a strong understanding of anatomy, physiology, and medical terminology, as well as the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes. Duties Conduct comprehensive assessments of patients to determine their physical therapy needs. Develop and implement personalized care plans based on individual patient evaluations. Utilize various therapeutic techniques including trigger point therapy, physiotherapy, and orthopedic rehabilitation. Provide education to patients and their families regarding treatment plans and home exercise programs. Document patient progress accurately in EMR systems and maintain detailed records of care provided. Collaborate with interdisciplinary teams to coordinate discharge planning and ensure continuity of care. Work with diverse patient populations including pediatrics, geriatrics, and individuals requiring acute care. Engage in community outreach programs to promote health awareness and preventative care. Experience Proven experience in physical therapy settings such as hospitals, clinics, or home health environments is preferred. Familiarity with discharge planning processes and the development of effective care plans. Knowledge of medical terminology, anatomy, and physiology is essential for effective patient assessment and treatment. Experience working with various patient demographics including outpatient services, inpatient rehabilitation, and community-based care. Background in specialized areas such as orthopedics or pediatrics is a plus. Proficiency in using EMR systems for documentation and patient management.
