Physical Therapist

Job Summary We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will have a strong background in patient care, with expertise in various areas of physiotherapy including acute care, outpatient, and home health settings. This role involves developing individualized care plans, providing effective treatment interventions, and ensuring optimal patient outcomes through evidence-based practices. Responsibilities Conduct comprehensive evaluations of patients to assess their physical abilities and limitations. Develop and implement personalized care plans based on patient needs and goals. Provide therapeutic interventions including trigger point therapy, orthopedics, and geriatrics. Collaborate with interdisciplinary teams to ensure coordinated patient care across inpatient, outpatient, and home health environments. Educate patients and their families on treatment plans, exercises, and self-management strategies. Maintain accurate documentation of patient progress using EMR systems. Participate in discharge planning to ensure continuity of care post-treatment. Stay current with advancements in physiotherapy practices and medical terminology. Qualifications Degree in Physical Therapy from an accredited institution. Valid state licensure to practice as a Physical Therapist. Knowledge of anatomy and physiology relevant to patient care. Experience in pediatrics, geriatrics, or orthopedics is preferred but not required. Familiarity with home & community care practices is a plus. Previous hospital or clinic experience is highly desirable. Strong communication skills and the ability to work collaboratively within a team environment.
