General Description The Physical Therapist at Evara Health plays a vital role in our multidisciplinary team by assisting patients in developing, recovering, improving, and maintaining the skills needed for daily living and working. They are instrumental in providing specialized care to patients with various musculoskeletal and neuromuscular conditions. The Physical Therapist collaborates with a multidisciplinary team to ensure holistic, patient-centered care, aligning with our mission to deliver quality healthcare services. Essential Duties & Responsibilities Patient Care & Treatment – Conduct thorough assessments, create personalized treatment plans, implement therapeutic interventions, and adjust care as needed to achieve optimal outcomes while educating patients and families on health maintenance strategies. Collaboration & Mentorship – Work closely with healthcare professionals for integrated care, mentor residents, students, and trainees, and support clinical staff in delivering high-quality, cost-effective healthcare. Compliance & Documentation – Maintain accurate, detailed patient records, and ensure strict adherence to HIPAA, HRSA, State, Federal, and accreditation standards, as well as organizational policies and procedures. Professional Development & Best Practices – Stay current with advancements in physical therapy, incorporating evidence-based practices into patient care to continually enhance quality and effectiveness. Minimum Qualifications Education and Experience: Doctor of Physical Therapy (DPT) degree from an accredited institution. A minimum of two years of professional experience in physical therapy, demonstrating a broad range of clinical skills in a healthcare setting. Exceptional communication and interpersonal skills to effectively engage with patients, families, and healthcare teams. Licenses, Certification or Registration: Valid and active state licensure as an Physical Therapist required. Basic Life Support (BLS) certification required.
