What You'll Do Conduct comprehensive initial evaluations and assessments and promptly report findings to the physician/provider and Clinical Manager Develop, implement, and update individualized plans of care based on patient needs and physician orders Perform skilled physical therapy interventions, procedures, measurements, evaluations, and diagnostic tests in the home setting Complete initial evaluations, treatment documentation, and re-evaluations every thirty (30) days; update plans of care as needed Complete comprehensive assessments and OASIS documentation accurately and on a timely basis, as applicable Observe, document, and report patient response, progress, and any significant changes to the physician/provider and care team Communicate and collaborate with physicians, providers, and interdisciplinary team members to ensure coordinated, high-quality care Provide education and support to patients and their families to promote safety, independence, and long-term succe
