Job Description
Inspire Hospice & Palliative Care is seeking a compassionate and dedicated Physical Therapist (PT) to join our interdisciplinary care team on a Paid Per Visit (PPV) basis. This role provides personalized therapy services to hospice patients in their homes, helping improve comfort, mobility, safety, and quality of life. Position Summary The Physical Therapist plans, organizes, implements, and evaluates physical therapy treatment programs based on physician referrals and patient assessments. This position works collaboratively with the Interdisciplinary Group/Team (IDG/IDT) to provide quality, patient-centered hospice care. Responsibilities Perform clinical assessments to identify patient needs and therapy appropriateness Develop, document, and revise individualized Plans of Care (POC) Complete timely clinical documentation, progress notes, and physician orders Provide therapy treatments for neuromuscular, neurological, and orthopedic conditions Educate patients and caregivers on transfer techniques and safe equipment use Utilize therapy equipment appropriate to patient diagnoses and needs Collaborate with the hospice interdisciplinary team to coordinate care Maintain infection control standards and safety practices Accept assignments consistent with education, experience, and competency Participate in continuing education and maintain licensure requirements Promote the mission and values of Inspire Hospice & Palliative Care Qualifications Education Graduate of a CAPTE-approved Physical Therapist Education Program Doctor of Physical Therapy (DPT) required ( Exceptions may apply for education/licensure obtained prior to 2009 or outside the United States ) Licensure Current state Physical Therapist license or compact PT license where applicable Experience Two years of experience in acute care or rehabilitation preferred Skills & Requirements Strong therapy and patient assessment skills Knowledge of durable medical equipment Excellent communication and interpersonal skills CPR certification preferred/required per hospice policy Reliable transportation and valid auto liability insurance required Work Environment & Physical Requirements Travel to patient homes and healthcare facilities within assigned territory Ability to lift, bend, push, pull, and stand for extended periods Ability to work in emotionally sensitive situations with terminally ill patients and families Exposure to varying weather conditions and healthcare environments Compensation Paid Per Visit (PPV) Flexible scheduling opportunities available Why Join Inspire Hospice?
