Active Indiana Physical Therapist license required to start the assignment. BLS certification required from AHA or ARC. Minimum 1 year of clinical experience in outpatient orthopedics or related settings. Experience treating post-op knee, hip, shoulder and low back/neck pain required. Experience with modalities and equipment including estim, ultrasound, Hivamat, laser, AlterG, or Vector is preferred. Experience using EPIC or comparable EMR preferred. Demonstrated ability to work 1:1 with adult ortho/neuro and post-op patient populations. Preferred: manual therapy skills. Assignment Details Facility Type: Hospital outpatient / clinic setting. Shift: Days. Shift Hours: 08:30–17:00. Weekly Guaranteed Hours: 36. Patient Ratio: 1:1. Required Certifications: BLS and valid state licensure. Manager: Erin Charters. Reporting location: YMCA, 3001 S Creasy Lane, Lafayette, IN 47905. Dress Code: dark gray scrubs. Unacceptable attire: open-toed shoes, denim, sweatshirts.
