Apply now and a recruiter will contact you with more details. Minimum Requirements PT degree (Bachelor’s, Master’s, or Doctoral) from an accredited program approved by the APTA. Active Pennsylvania Physical Therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Home health experience preferred. This role will function as the primary PT for the agency and requires completion of SOCs, evaluations, reassessments, discharges, establishment of POCs, supervision of a PTA, caseload management, and daily patient outreach and scheduling. Experience with electronic documentation systems desirable. Knowledge of HCHB and OASIS is preferred. Ability to travel within York County and western Adams County as part of regular patient visit coverage. Valid driver’s license and ability to document mileage between patient visits (mileage reimbursement provided per client policy). Assignment Details Facility Type: Home Health. Shift: Days. Shift Hours: 8:00 AM – 4:00 PM (client indicates 8:00 AM – 4:30 PM for scheduling; schedule is Monday through Friday). Weekly Hours: Typically 32–40 hours per week; weekly guarantee noted by client is 32–40. Weekend/On-call Requirement: None. Primary responsibilities: serve as primary PT, perform SOCs, evaluations, reassessments, discharges, establish Plans of Care, supervise PTA, manage independent caseload, and perform daily patient outreach and scheduling.
