Apply now and a recruiter will reach out with more details. Minimum Requirements Bachelor of Science, Master of Science, or Doctor of Physical Therapy from an APTA-accredited program - Required. Current Missouri Physical Therapist license or ability to obtain state license prior to start - Required. Proof of current CPR certification - Required. Current driver’s license, reliable transportation, and vehicle insurance - Required. 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting - Required. Home health experience - Preferred. Knowledge of durable medical equipment and ability to order and adapt equipment for home use - Required. Ability to perform clinical assessments, prepare and revise Plans of Care (POC), and document progress notes and phone orders - Required. Ability to perform transfer training and instruct patients/families in safe use of walkers, wheelchairs, canes, braces, and prosthetic devices - Required. Ability to perform prolonged standing, walking, lifting, bending, pulling and pushing as job duties require - Required. Meets mandatory continuing education requirements and demonstrates professional growth and competency - Required. Ability to work a flexible schedule and travel locally as part of job duties - Required. Assignment Details Facility Type: Home Health. Assignment Type: Travel, Temporary Contractor. Start and End Window: assignment begins in April 2026 and ends in July 2026. Clinical Focus: Evaluation and treatment of neuromuscular, neurological, and orthopedic disorders in the home setting. Documentation Expectations: Complete and revise Plan of Care, progress notes, and other clinical documentation per agency policy. Equipment Responsibilities: Order and adapt rehabilitative equipment for patient home environments. Mobility Requirement: Must have reliable transportation; local travel to patient homes required.
