Active Pennsylvania Physical Therapist license required to start the assignment. BLS certification from AHA or ARC may be required. Experience with home health environments preferred, including evaluations, start of care, OASIS visits, and return/supervisor visits. Willingness to travel within assigned coverage areas including Somerset County, Cambria County, Northern Cambria, and Indiana County. Ability to meet productivity expectations (30 points) and weekly hour expectations up to 40 hours (32 hours guaranteed stated by client). Professional dress per client policy: royal blue or black scrubs, or khakis and polo shirt. Valid driver’s license and ability to document mileage for reimbursement between visits. Preferred: prior home health documentation and OASIS experience. Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Details: Days 08:00–16:00 (8:00am–4:00pm). Weekly Hours: Weekly hour guarantee up to 40 hours; client indicates 32 hours guaranteed. Weekend/On-call: Weekend/on-call requirement discussed with client; coverage every 8 weekends indicated. Coverage Area: Somerset County with additional coverage across Cambria, Northern Cambria, and Indiana County. Mileage reimbursed between visits. Primary responsibilities: patient evaluations, start of care, OASIS visits, and return/supervisor visits.
