Apply now and a recruiter will reach out with more details. Minimum Requirements Bachelor‘s, Master‘s, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Illinois Physical Therapy license required to start the assignment. BLS certification from AHA or ARC may be required. Outpatient clinic experience required. Experience with evaluations, plan of care development, and functional treatment programs preferred. Documentation proficiency required, including goal writing, functional quality indicator assessment, letters of medical necessity, and electronic charge entry. Experience supervising and delegating to discipline-specific therapy staff and participating in student clinical education preferred. Ability to comply with state practice act, maintain licensure, and complete state-mandated continuing education. Assignment Details Facility Type: Outpatient. Shift Pattern: 5x8 schedule.
