Harmony Home Health seeks a Full Time, Part Time, and Per-Diem Physical Therapy Assistant to join its dynamic team of healthcare professionals in the Santa Clara County and San Benito County areas.Harmony Home Health is an Agency that is Medicare-Certified and State-Licensed.The responsibilities of the Physical Therapy Assistant include:Provides direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care.Treats patient to relieve pain, develop or restore function, and maintain maximum performance.Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage.Observes and reports to the physical therapist and the Organization personnel the patient's reaction to treatment and any changes in patient's condition.Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient's clinical record in prescribed time frames.Participates in Organization Home Care Team Members meetings and Organization Inservice Education Program.Performs other duties as assigned.Minimum qualifications of the Physical Therapy Assistant include:Active Physical Therapy Assistant License in the state of California - at least 1 year professional PTA experience preferred Home Health (PTA) experience is a plus Able to prioritize and manage time effectively.Highly organized and detail-oriented.Able to follow verbal and written instructions Able to communicate effectively.Bilingual communication a plus but not required.Dependable Transportation, Current Driver's License and auto Insurance.Current CPR Ability to perform the physical aspects of the position.Complete TB test and Physical Examination We offer competitive visit rates for our clinicians.We know you can help us make a difference in providing care centered on enhancing the lives of seniors and allowing them the independence of remaining at home in their healthiest state of well-being.If you have a passion for working among the senior population and the ability to blend healthcare and hospitality service philosophies, we would love to receive your resume!
