The Nephrology Physician APP will be a core member of the BWH Department of Medicine and key constituent of the Nephrology Division. The nature and scope of practice of the APP for Nephrology is to assess, diagnose and treat health problems of patients across the continuum of care, in collaboration with the Nephrology attendings. The practice will encompass outpatient care for patients with CKD and ESRD. Candidates for the position should have a strong interest in acute care nephrology and, ideally, experience and training in clinical nephrology. The nephrology APP will report to the BWH Clinic Director. The nephrology APP will work very closely with all BWH nephrology attending physicians to develop the necessary clinical skills in a highly collaborative and supportive environment. Principal Duties And Responsibilities Indicate key areas of responsibility, major job duties, special projects and key objectives for this position. These items should be evaluated throughout the year and included in the written annual evaluation. The APP works closely with the nephrology physicians, inpatient physicians and advanced practice professionals, residents and nursing staff to develop and coordinate a plan of care designed to meet the physical, psychological and social needs of patients and their families. Strong interpersonal, organizational and communication skills are required. Knowledge of current literature and a commitment to continued learning are also required. The mission, vision and core values of the Brigham and Women’s Hospital (BWH) are incorporated into all areas of practice. The BWH nephrology APP will also be an integral partner with the other BWH advanced practice professionals. As a member of the BWH Nephrology Division, the PA shall be responsible for the following duties: 3 full days of outpatient clinic: see patients in outpatient clinic setting the APP will see patients with CKD/ESRD for General and Interventional Nephrology (5-7 patients a day) see patients for post discharge follow up in need of expedited evaluation. provide education for patients approaching ESRD, encompassing renal transplantation hemodialysis (HD), peritoneal dialysis (PD), and home HD options Facilitate kidney replacement (KRT) planning for pre-dialysis CKD patients, in collaboration with Renal Transplant, Transplant Surgery, and Interventional Nephrology provide protocolized care for early-stage CKD patients and diabetic patients, both with and without proteinuria collaborate with attending physicians in the shared care of Renal clinic patients 1.5 days dedicated to administrative responsibilities, care coordination, and interdisciplinary collaboration. General Duties Work collaboratively and function as an efficient member of the BWH nephrology division Assist in the coordination of care for patients with the primary care teams and other sub-specialists involved in the care of patients Provide education to patients and families Document in the medical record as appropriate and communicate observations and findings to the attending physicians Monitor environment of care and comply with regulatory agency standards Ensure that his or her clinical practice is consistent with physician assistant policies, procedures and standards Collaborate with the Medical Director in protocolizing patient care algorithms and in educating Renal providers on said protocols Participate in QA/QI projects as appropriate Summary Job Summary The Physician Assistant (PA) is a licensed provider. The PA is responsible for the assessment and management of various populations of patients, medical and/or surgical, including diagnostic and therapeutic interventions, development of appropriate plans of care and ongoing evaluation.
